Essential Guide to Under-Eye Fat Repositioning by Type: Know Why It Can Fail
Covers the four under-eye fat types that make you look tired, the right treatment for each, and when lower blepharoplasty is needed.

Hello, everyone! This is Dr. Jang Jun-cheol, director of Dreaming Plastic Surgery Clinic.

Just as a common cold can cause a severe cough in one person and a high fever in another,
under-eye fat that makes you look tired and older also comes in four different types.
Let me walk you through the characteristics of each type and how to address it.

✅ This is when the fat bulges out while the tear trough beneath it is deeply hollowed, creating a sunken look.
The boundary between the bulge and the hollow is so distinct that it makes you look more worn out and older.

✅ Some people have only fat bulging under the eye.
There's no particular hollow, but the fat puffs out on its own, casting shadows beneath the eye and making you look tired.

✅ Some people have very little protruding fat,
but the entire under-eye area is deeply sunken, making it look like dark circles.
It feels like a dark, deep pit beneath the eyes.

✅ The last type has a bit of under-eye fat, but the trough beneath it is so deep that
the entire under-eye area looks hollowed out. People like this tend to look like they have the most severe dark circles.

How about it? Does any of these sound exactly like your situation?
So many different under-eye concerns!
Under-eye fat repositioning can be used for all of these cases, but the surgical approach varies slightly from person to person.

All of these are performed through the inner conjunctiva, so recovery is quick and there are no visible scars.
The classic case for under-eye fat repositioning is when there is both bulging and hollowing.
The protruding fat is moved into the hollowed trough, creating a smooth, even under-eye area.

When only the fat bulges out, under-eye fat removal is performed to take out part of the protruding fat.
When there is only hollowing, restoring the missing volume is generally the key.

Lightly filling the hollowed area with micro autologous fat grafting or hyaluronic acid
can make the overall look much softer.
Finally, when the hollowing is more pronounced than the bulging, the under-eye area is flattened through repositioning,
and additional fat grafting or hyaluronic acid filler is combined as needed to restore volume.

Even within under-eye fat repositioning, the approach varies slightly depending on the condition of the under-eye area. That's why we carefully examine the under-eye area during consultations and recommend the most suitable method.
Now, here's an important point.

Some people feel it's a bit of a waste to do it now and prefer to wait a few more years.
Unfortunately, the problem gets worse over time, and the surgery may also become more extensive.

If under-eye fat protrusion and hollowing continue to progress, the skin under the eye itself stretches and loses elasticity.
When the skin sags too much, under-eye fat repositioning alone, which only rearranges the fat, reaches its limits.
In such cases, lower blepharoplasty should be considered, which removes the excess skin and repositions the fat.

Lower blepharoplasty involves an incision along the lower lash line to remove the sagging skin,
and then repositions the fat, just like under-eye fat repositioning surgery.

Because the surgical area is wider and more complex, the surgery costs more,
and since the skin is incised, recovery takes longer and swelling and bruising may last longer as well.

So when under-eye bulging or hollowing is not severe,
it's more beneficial in many ways to choose under-eye fat repositioning alone if it can provide enough improvement!
That's because the cost is lower, recovery is shorter, and you can return to your daily life more quickly.
Today, we've looked at the various types of under-eye concerns and the tailored solutions for each.
Tired-looking, aged under-eyes can be neatly resolved in just one hour.

This has been Dreamcatcher Dr. Dream, here to thoroughly assess your under-eye area and help you find a bright, fresh look.
Thank you.

Frequently Asked Questions
Is under-eye fat repositioning the same for every under-eye concern?
No, the approach varies depending on the condition of the under-eye area. Cases with both bulging and hollowing are the classic case for under-eye fat repositioning, while cases with only protruding fat are treated with under-eye fat removal, which removes part of the fat.
How is it treated when only the fat under the eye bulges out?
It is described as under-eye fat removal, in which part of the protruding fat is removed. This applies to cases where only the fat bulges out without any particular hollowing, and it is explained that this can cast shadows under the eyes and make a person look tired.
What treatment is used if the under-eye area is only hollow?
The approach described is to lightly fill the hollowed area with micro autologous fat grafting or hyaluronic acid, among other options. Restoring the missing volume is considered important, and doing so is said to make the overall look much softer.
When is under-eye fat repositioning alone not enough?
It is explained that this is when the skin has sagged significantly and repositioning the fat alone has its limits. In such cases, lower blepharoplasty should be considered. It is also noted that if the bulging and hollowing continue to progress, the skin may stretch and lose elasticity.
How is lower blepharoplasty different from under-eye fat repositioning?
Lower blepharoplasty is a procedure in which an incision is made along the lower lash line to remove sagging skin, and the fat is then repositioned. Because the surgical area is wider, it costs more, and the incision may mean a longer recovery period, with swelling and bruising lasting longer.
How is under-eye fat repositioning performed?
It is performed through the inner conjunctiva, and it is described as offering quick recovery with no visible scarring. The basic principle is to move the protruding fat into the hollowed trough, smoothing the under-eye area so it looks even and seamless.
